Chronic Obstructive Pulmonary Disease (COPD) in itself presents significant and increasing burdens on global health services. Reports estimated that by 2030 COPD will be the 4th largest cause of global mortality (World Health Organisation, 2007). Many patients diagnosed with COPD may also suffer from closely-related chronic diseases including heart failure, diabetes and depression. These co-morbidities add further complexity and cost to public health services, and significantly lower the quality of life experiences and expectations for patients suffering with COPD.
Patients therefore have complex ongoing medical care needs, often involving multiple therapies as well as self-management responsibilities. Public health services are under increasing pressure to deliver integrated care pathways which optimise the quality and timeliness of medical intervention and reduce costs brought about by prolonged hospitalisation.
WELCOME project aims to create a technology solution enabling step-change in the integrated care of, and self-management by, patients suffering from Chronic Obstructive Pulmonary Disease (COPD) and associated chronic conditions (comorbidities) including Congestive Heart Failure (CHF), Diabetes and Anxiety and Depression.
- Between 2013-2017 will be developed, tested, validated a fully integrated personalised technology solution
- The technology solution combines a smart vest with sensors to measure and monitor specific COPD patient indicators. Combined with other patient and environmental information, real-time data can be automatically uploaded to highly secure cloud-based patient medical records.
